1. Application of the child does not guarantee Admission.

2. If any of the particulars mentioned in this form are found incorrect at any time, even after the child is admitted into the school, his/her admission will be cancelled and fee will not be refunded.

3. Attach the photocopy of Aadhar card of ward, Father and Mother.

4. Application fee ₹700/- (Non-refundable and non-adjustable) draft to be submitted to the office along with completed form. Draft in favour of ST THERESA SR. SEC.SCHOOL, payable at KATHGODAM.

AGE CRITERIA

• For Nursery: A child should be completing 3 years by 01/04/2026

• For LKG: A child should be completing 4 years by 01/04/2026

• For UKG: A child should be completing 5 years by 01/04/2026

5. Photograph of applicant child should be clicked after October 2025. Name and Date of photo taken should be mentioned on applicant photograph.

6. Valid E-mail ID and Mobile no. of the parent required.

7. Applicant Child’s Date of Birth Certificate: (Certificate should bear the Child’s Name, Father’s Name and Mother’s name.)

8. Attach the photocopy of Aadhar Card of ward, Mother and Father.

9. Blood group of the Child.

10. Application Form and the original Birth Certificate to be brought at the time of Interaction (Interview).
